What clinical services are available through Liberty Day Supports?
The Day Treatment Program includes the following clinical supports: Physical Therapy, Occupational Therapy, Nursing, Speech and Language Therapy, Psychological and Behavioral Supports, Social Work, and Nutritional Services.
What if I am in a Liberty Day Support Program that does not offer clinical services and I am in need of these services?
Liberty works with a number of providers of medical and clinical services in the community. In addition, Liberty operates
an Article 28 clinic, known as New Dimensions in Health Care, which offers a wide variety of clinical and medical services, and specializes in providing these services to people who need special accommodation.
Is transportation provided to and from Liberty’s Day Support Programs?
Liberty provides transportation services to and from the Day Support Program.
Do I have to pay for Liberty Day Supports?
Liberty’s Day Support Programs are funded in full through Medicaid. Liberty has staff able to assist in determining
Medicaid eligibility. If it is deemed that a person is not eligible for Medicaid, there are options for private pay.
Are lunches and snacks provided as part of the program day?
Nutritious lunches and snacks are provided each day to individuals who attend Liberty’s Day Supports Programs.
There is no cost to individuals in the program for this service.
As a family member, how much involvement can I have in the program?
Liberty believes that each person should be at the center of their program planning and program implementation.
Family members and close friends play a significant role in supporting the consumer in the development of their plans
and overall decision making. Family and friends are always welcome to visit and observe the activities of the Day
Supports Program.
