FAMILY SUPPORT SERVICES
Some of the Family Support Programs outlined here are
funded in their entirety or in part by Liberty Foundation, Inc.

All families need time apart to balance the normal pressures of life. This is particularly true of families caring for an individual with a disability. In response to this need, Liberty has developed a wide range of family support programs designed to assist families while providing enjoyable, worthwhile experiences for individuals with disabilities.

The following is an outline of the many programs offered through Liberty's Family Support Services department:

Medicaid Service Coordination: Professional staff help families in planning and accessing needed services.

 

Home & Community-Based Waiver Program: Liberty staff can help obtain resources for adaptive equipment, home renovations, or in-home assistance so individuals have the opportunity to be more independent and productive in their home.

Basketball

Respite Programs: A minimum of 120 hours of respite is available to any family of an adult or child with a developmental disability - at no cost. Extra hours, up to 150, are available to families with higher levels of need or who meet other eligibility requirements. A voucher program gives families the option of hiring someone they know to provide respite - with an hourly stipend reimbursed by Liberty.

When a family crisis occurs, Liberty will provide emergency respite as needed at no cost.

Recreation Program: A variety of athletic, social and special events are scheduled on evenings and weekends throughout the month. Activities include bowling, swimming, cooking, arts & crafts, dances and involvement in Special Olympics sports training and events. Transportation to and from recreation programs is provided.

 

After School Program: Liberty offers an after school program to individuals of school age with developmental disabilities and their school age siblings. This is available to children ages 5-21 in the Amsterdam School District only.

School Recess Program: is available to any child with a developmental disability residing in Montgomery County. Trained staff provide enjoyable, learning activities during planned breaks throughout the school year.

Advocacy: Trained staff are available to assist in accessing needed services for you or your family member with a disability. Areas of focus include but are not limited to: educational services and benefits.

Hispanic Outreach: Bilingual staff are available to assist families in accessing services and supports in the community and at Liberty.

Information & Referral: Liberty can provide a variety of information regarding the nature of disabilities, as well as service options, and support groups. Assistance is also available in accessing needed services when they are identified.
Guardianship: If you are considering obtaining guardianship for a family member with a developmental disability, Liberty offers assistance in the process, including financial aid.
Medical Transportation: Liberty can provide transportation and needed supervision to medical appointments for individuals with a developmental disability. Specialized transportation for individuals confined to wheelchairs is available.
Liberty's Family Support Services are available to any family in Montgomery County
who cares for a child or adult with a developmental disability.
