E-Badges Application

NADSP Application Form

Thank you for your interested in Liberty ARC’s NADSP E-Badges credentialing program. The application for the Fall 2025 cohort is below.

I agree that I have met the standards, listed below, to apply for the NADSP E-Badges Program.(Required)
In order to be selected the participant must be in good standing with the agency; not have received a written warning the last 6 months; have a minimum of 1 year DSP experience; and model the behaviors which exemplify our Values: Quality, Respect, Integrity, Teamwork, Resourcefulness, and Person Centered.
